Request for MISD Service Record
The Service Record Request Online Form is the required document for all former and existing MISD employees to request an official service record from the District. Please complete and submit the form online or print and fax it to Amber Konchalski at 817-473-5488.
- Employees leaving in the current school year: Please allow up to 30 days after your final work date for your request to be processed.
- Current/Former Employees: Please allow at least five business days for processing.
- Employees leaving the District at the end of the school year: Service Records will be processed in July.
If the service record is required for additional certification, please indicate in the text box at the bottom of the Records Request Form.
NOTE: It is MISD policy to prepare one original service record. If MISD has previously provided an original service record for you in the past, please contact your most recent district to obtain the original.
