Mansfield Independent School DistrictAthletic Participation Packet
All students trying out for a school sport are required to complete and submit the following prior to the start of the school year:
- Sports physical
- Physical exams must be signed by a licensed physician, physician assistant (PA), advanced nurse practitioner or doctor before it is turned in to the coach or athletic trainer.
- Medical history form
- Athletic participation packet
Student-athletes will not be allowed to participate in any athletic classes, practices or events until all necessary forms are completed and processed with the MISD Athletic Department.

Participation in athletics comes with an inherent risk of potential physical injury. Mansfield ISD is not responsible for any medical expense incurred while participating in athletics.
