Staff Vaccination Clinics

Passport Health/Staying Healthy Medical Services will be providing on-site vaccination clinics for Mansfield ISD employees starting Oct. 13.Employees (and their covered dependents) who have insurance through the district can receive immunizations, most at no cost, during this annual staff vaccination clinic provided by Passport Health/Staying Healthy Medical Services. Review all insurance options and information on how to register below.

Accepted Forms of Insurance

In addition to district provided insurance, other insurance plans that are accepted and may be partially or fully covered are Aetna, Blue Cross Blue Shield (BCBS), Cigna, Humana, United Health Care, Medicare Part B and Scott & White.

If you do NOT have accepted insurance providers, you can still purchase any vaccine at the following costs. Accepted forms of payment are cash and checks made payable to Staying Healthy Medical Services.

Registering for your Vaccinations

Employees and their covered dependents are eligible to go to any location for their vaccines from Oct. 13 through Nov. 2. (Two booster shot/makeup clinics are being offered on Nov. 28 and Dec. 1.)

To sign up for your vaccines, please go to the Passport Health MISD Flu Clinic Signup webpage.

After you sign up, you will receive a confirmation with a link to a consent form. On the day of your vaccination, you MUST bring the consent form (available after your online registration is confirmed), your insurance card (to receive your vaccinations at no charge) and your driver's license. Electronic forms of cards cannot be accepted.

If you are paying for your vaccinations or the B-12 shot, cash and check (made payable to Staying Healthy Medical Services) are acceptable forms of payment.
