Staff Vaccination Clinics
Passport Health/Staying Healthy Medical Services will be providing on-site vaccination clinics for Mansfield ISD employees Oct. 15 - Nov. 2, 2021.Employees (and their covered dependents) who have insurance through the district can receive immunizations, most at no cost, during the annual staff vaccination clinic provided by Passport Health/Staying Healthy Medical Services. Review all insurance options, our vaccination schedule and information on how to register.
Accepted Forms of Insurance
In addition to district provided insurance, other insurance plans that are accepted and may be partially or fully covered are Aetna, Blue Cross Blue Shield (BCBS), Cigna, Humana, Medicare Part B, Scott & White and United Health Care.
If you do NOT have accepted insurance providers, you can still purchase any vaccine at the following costs. Accepted forms of payment are cash and checks made payable to Staying Healthy Medical Services.

*The Moderna COVID-19 vaccine will be available for participants ages 18 and up. The vaccine will be given as dose 1, 2 or 3 as long as it has been 28 days between doses. All doses must be from the same vaccine.

Register Online for your Vaccinations
To sign up for your vaccines, please go to the Passport Health MISD Flu Clinic Signup webpage. After you sign up, you will receive a confirmation with a link to a consent form.
On the day of your vaccination, you MUST bring the consent form (available after your online registration is confirmed), your insurance card (to receive your vaccinations at no charge) and your driver's license. Electronic forms of cards cannot be accepted.
If you are paying for your vaccinations or the B-12 shot, cash and check (made payable to Staying Healthy Medical Services) are acceptable forms of payment.
If you have questions about a particular vaccine, you may direct your questions to the nurse on the day of the clinic or contact a school nurse in advance.
Passport Health/Staying Healthy Medical Services will be administering vaccines at the following district buildings and schools from Oct. 15 through Nov. 2, 2021.
Feel free to attend the clinic that best fits your schedule. Online registration is highly recommended.
Makeup & Booster Clinics | Nov. 2 and Nov. 29
Did you miss the clinic held at your building? Passport Health/Staying Healthy Medical Services will be administering vaccines and booster doses, like COVID shots, at the makeup clinic on Tuesday, Nov. 2 from 4:00 p.m. - 5:30 p.m. AND Monday, Nov. 29 from 3:00 p.m. - 5:00 p.m. at the MISD Health Services department (1920 N.
