MISD Athletics Physicals for grades 7-12 will be held at Methodist Mansfield Medical Center (2700 E Broad St, Mansfield, TX 76063) on Saturday, April 29 from 9 a.m. to noon. The cost for a physical is $10.

Athletic physicals are for those who want to participate in athletics during grades 7-12. Physicals are a requirement for participation in our athletic programs.  They are valid for one school year, including summer strength training and conditioning workouts.

In addition, Methodist Mansfield will offer free echocardiogram screenings. These free screenings are available by appointment only. Please call 682-242-6401 for more information on the free screenings.

All participants will need to fill out and bring the following forms:

• Echocardiogram Screening Patient Consent Form
• Echocardiogram Patient Screening Evaluation
• Physical Evaluation Medical History Form
