| To work directly/indirectly under the supervision of the Coordinator of Student Health Services and/or the Registered Nurse on that campus. To provide assistance in all aspects of the duties and responsibilities of the registered nurse or Coordinator for the benefit of the school health program. The LVN has no responsibilities as the sole heath program provider. | |||

| Graduate of an accredited school of Vocational Nursing Current Texas license issued by the Board of Vocational Nurse Examiners Certified by Texas Department of Health to conduct vision, hearing and scoliosis screenings, in last five years. CPR Certification in last two years health care provider card) |

| Major Responsibilities and Duties: | |||
| 1. Works under the direct or indirect supervision of the registered
nurse on same campus or Coordinator of Student Health Services. 2. Performs and/or assists with state required vision, hearing and scoliosis health screenings. Also performs and/or assists with dental screenings and height and weight measurements. 3. Records health screening results on the student health record in a timely manner. 4. Performs or assists the registered nurse in referring students who do not pass the state required health screens. Keeps accurate records of referrals and outcomes of referrals as directed by the registered nurse or Coordinator. 5. Maintain student health records and student emergency cards on enrolled students. 6. Maintain immunization records on enrolled students. Performs or assists the registered nurse in checking immunization records to verify compliance with current state laws. Maintains an original immunization document in each health file. 7. Performs and/or assists the registered nurse in notifying parents of delinquent immunizations and required boosters. Keep accurate records of delinquent/required boosters. Follows up to assure immunizations are complete by the specific due date. 8. Performs minor first aid. 9. Calls parents to report information regarding illnesses/injuries during school hours. 10. Administers medication under the direct or indirect supervision of the registered nurse or Coordinator when medication fulfills district requirements and district procedures for administration of medications are followed. 11. Completes referrals, advises parents of the need for referral ad monitors results of referrals on student health record, as directed by the registered nurse or Coordinator. 12. Reports information to registered nurse or Coordinator on an as needed basis. 13. Performs tube feedings urinary catheterizations and other delegated procedures after training and completing skills checklist as monitored by registered nurse or Coordinator. 14. Maintains accurate records of student visits to the health clinic. 15. Consults with registered nurse or Coordinator if unsure about procedures or emergency situations. 16. Documents communicable diseases per district policy. 17. Does not offer medical opinions concerning a student's illness or injury (does not diagnose). 18. Maintains confidentiality of all health information and records 19. Understands and follows district policies and procedures regarding school health. 20. Performs clerical duties related to school health as directed by the registered nurse or Coordinator. 21. Performs other duties as assigned by the registered nurse or Coordinator. 22. Do requisition for supplies and equipment needed to maintain clinic inventory. |
