For those employees (and their covered dependents) who have insurance through TRS ActiveCare – Blue Cross/Blue Shield, your flu shot as well as other adult immunizations are covered at 100% under your plan. If you do NOT have TRS ActiveCare, you can still purchase the vaccines at the costs below:

NEW THIS YEAR: Register for your vaccinations online!!! To sign up for your vaccines, please go to: http://www.passageware.com/FluSignUp/MISD/. You will then be able to choose a location and time for your vaccinations. Please fill out the entire form including your name, date of birth, email and phone number.
After you sign up, you will receive a confirmation email. There will be a link for you to print out the consent forms for your vaccinations. This will save you time on the day of the flu clinic.
On the date of your vaccinations, you must bring your TRS ActiveCare insurance card and driver's license to receive your vaccinations at no charge.
If you have questions about a particular vaccine, see your school nurse.
2011 Vaccination Schedule - Passport Health will be administering the Flu Vaccine and other Vaccines at the following departments in the designated areas. For all campuses, contact the school nurse for locations. Feel free to attend the department or campus that meets your schedule.

Passport Health will be administering vaccines requiring more than one dose (Hepatitis B) as well as any other vaccinations you missed in October on the following dates. Feel free to attend the campus that meets your schedule.
