Enrollment & Registration

All students new to MISD must complete the forms below and bring them to the campus at their designated registration time.

Enrollment for your student(s) takes place at the campus of assignment. Your campus of assignment is determined by the parent’s or legal guardian’s permanent place of residency.

If you need assistance in determining which campus your child is zoned to attend, you may call the Department of Transportation (817) 299-6060 or use the Web Query System to find your school.


To enroll your child you will need the following documents:

  1. Parent/Legal Guardian’s Government Issued Photo Identification
  2. Proof of Residency in the form of a current (within last 30 days) electric, gas, water bill or lease agreement. (No other documents will be accepted).

  4. Student’s Shot Record
  5. Student’s Certified Birth Certificate

If you are unable to provide sufficient proof of residency because you are currently living with someone who is the actual MISD resident and there is no electric, gas, water bill or lease agreement in your name, the MISD resident will need to complete an Affidavit for Proof of Residency stating that you live in his/her home. This form is available at the campus and also at the Department of Student Services. Submit the completed affidavit with all other enrollment documents at the campus of assignment. Upon approval at the campus level, you will be allowed to proceed with the enrollment process.

Families who are in the process of building a home and who are not in possession of the home by the first day of school must take their sales contract and a letter on letterhead stationary from their builder specifying a move-in/completion date to Student Services, 609 E. Broad. For the student to be eligible to enroll in MISD, the home must be completed and the family must reside in the home by the 45th day of school. (October 27, 2009). These items will need to be presented to the Director of Campus Support for verification. Upon approval at the district level, you will be allowed to proceed with the enrollment process at the campus.

Within 30 days of closing date for a newly built or purchased home, satisfactory proof of residence in the form of water, gas or electric bill addressed to the student’s parent at the new address must be provided to the campus.
 
Students will be withdrawn if proof of residence is not provided within 30 days of the closing date listed on the sales contract/letter from builder.

General Intra-District Transfer Requests | Deadline April 1

In order to be considered, transfer requests for the upcoming school year must be received in the Office of Student Services (609 East Broad St.) by April 1. The decision of the Director of Campus Support shall be considered final. Transportation is not provided for students who obtain an intra-district transfer. A student transfer may be requested only one time per school year.

(PLEASE NOTE: This form CANNOT be used for Employee Options)

Employee Option Transfer Requests | Deadline April 1

Full-time District employees may request transfers from their home attendance zone to the attendance zone for the campus in which they work provided that space is available. Detailed information can be found in the Employee Option Transfers Letter pdf

New or renewal requests for the next school year must be filed in the Office of Student Services by April 1 of the current school year or within 30 days of a new campus assignment. The form can be faxed, dropped off, mailed or sent by inter-office mail.

Verification of approval will be sent to you by intra-district mail. The approval form should be taken to the campus upon enrollment, schedule pick-up etc.

Pre-Kindergarten Eligibility

A child who is four (4) years of age on or before September 1 of the current school year is eligible for a pre-kindergarten program if the child meets one of the qualifiers listed below:

  1. Unable to speak and comprehend the English language;
  2. Economically disadvantaged (qualifies for free or reduced lunches);
  3. Homeless;
  4. The child of an active duty member of the armed forces, including
    state military forces or a reserve component of the armed forces, who
    is ordered to active duty (must be on active duty the first day of
    school);
  5. The child of a member of the armed forces, including state military
    forces or a reserve component of the armed forces, who was injured or
    killed while serving on active duty; or
  6. The child is or ever has been in the conservatorship of the Department of Family and Protective Services following an adverse hearing. This also
    includes children who have been adopted or returned to their parents.
