Previous Athletic Participation Forms

Any student/athletes who has participated for another high school must have an approved P.A.P. on file before he/she can compete for Timberview H.S. this season.

To begin the process, please use the online form on the UIL Athletics site (link below).

Parent/student will complete most of the first page and a small portion (top & section I) of the second page.

Once the form is printed, please add required signatures and get the form to Coach Button.  We'll take care of contacting the former school for their part, fill in our section, and send it off for approval.

Class of 2013 - If you're thinking about swimming in college, please visit the counselor's office to sign paperwork that will allow them to release transcripts to schools you're interested in.  You'll want to take the SAT and/or ACT and sign up with the NCAA Registration Center .  Helpful links are on our blog (unofficial site) (leaving MISD servers) here.

Paperwork - All athletes should have completed MISD Athletics Participation Packet on file. You can print one here.  Outdated and/or incomplete paperwork (lacking signatures, notarization, etc.) will not be accepted.

Physical - Part of the paperwork requirement is a completed physical.  Physicals can be scheduled through your family physician.

Dry-land Clothes - Shirts are $5 each and shorts are $10 per pair.  It's recommended that you have two (2) of each.

Swimming Equipment - Below is our equipment list.  The D&J swim shop at the natatorium is open on weeknights.  There are also several good online swim equipment sites.
